Permanent Jewelry Waiver and Liability Disclaimer

1. Procedure Disclosure

I acknowledge that permanent jewelry involves the custom fitting and welding of fine chain directly onto the wrist, ankle, or neck, creating a “claspless” and continuous piece. I understand that while the welding process is quick and typically painless, it involves the use of a small arc welder and sparks may be emitted during the process.

Protective materials will be used to shield my skin from the welding area, and I have the option to use protective eyewear provided by L T Denny Jewelers.

2. Risks and Safety

I understand that although safety precautions are in place, there are inherent risks associated with this service, including but not limited to:

• Minor burns or skin irritation

• Allergic reactions to metals

• Accidental snagging or breakage of the jewelry

I affirm that I do not have any known sensitivities to gold or other metals being used, or I have disclosed them prior to the service.

3. Removal and Medical Procedures

Permanent jewelry is designed for everyday wear but can be removed at any time with scissors or wire cutters. In case of medical emergencies (e.g., surgery, MRIs, injuries, etc.), I understand it is my responsibility to inform medical personnel and have the jewelry removed if necessary. L T Denny Jewelers is not responsible for any resulting medical complications or delays.

4. Warranty and Repairs

L T Denny Jewelers guarantees the weld point of your permanent jewelry for 30 days from the date of service. If the weld comes undone within this period, we will re-weld it at no charge. After 30 days, a re-weld fee may apply.

We are not responsible for lost chains, broken chains due to excessive tension, or damage caused by misuse, accidents, or neglect. We recommend treating your permanent jewelry with the same care as fine jewelry.

5. Age Requirement

All clients must be 18 years of age or older, or have a parent or legal guardian present to sign this waiver and be present for the service.

6. Liability Release

I voluntarily agree to receive permanent jewelry from L T Denny Jewelers and release and hold harmless L T Denny Jewelers, its owners, employees, agents, and affiliates from any and all liability, claims, demands, damages, or causes of action relating to or arising out of the permanent jewelry process or my participation in this service.

7. Media Consent (Optional)

I give L T Denny Jewelers permission to photograph or video the jewelry process or finished product for use on their website, marketing materials, or social media.
